    Parent Permission Form

Students at Muenster ISD are eligible to gain access to the K-12 eBook collection provided by the Education Service Center Region XI.  EBooks are checked out via the Follett Shelf on a student’s compatible device such as: a smartphone, tablets (browser based or app), or PC computer.  It is a true K-12 collection that contains 4836 Preschool through Young Adult books, along with some Adult Fiction and Non-Fiction titles. Feel free to use the link on our website to browse the collection. Book choices may be narrowed down by using the provided filters (i.e. Interest Level & Reading Level). Because some materials may be unsuitable for younger readers, we must have parent permission before we set up an eBook account for our Elementary and Junior High Students. Students may checkout/download books for a 5 day period. All accounts will be deleted at the end of the school year.

If you would like your child to have access to this collection please print this form, fill it out, and send it to school with your child.  Completed forms should be given to Mrs. Eldridge in either the Elementary or JH/HS Library.  Once the Tech Support at Region XI creates the account, Mrs. Eldridge will give your child their username and password.
